Our Policies

Cash patients - Payment for all services are due in full at the time of your visit, this includes payment for any labs or procedures that are done during your visit.  Most new patient office visits start at $200.00, any testing/procedures are in addition to that price.  Established patient visits start at $135.00, any testing/procedures are in addition to that price.

 

Commercial Insurance - If you have a copay, it is due at the time of service, we do not bill for co-pays.  We file primary insurances, however, we do not file secondary or tertiary insurances claims.  You will need to pay what the primary insurance says is due and file any secondary or tertiary insurance claims for reimbursement directly to you.  With high deductible plans you will be asked to pay an estimate of your responsibility - minimum of $100.00.

 

Medicare and Medicare Advantage Plans - If you have a copay, it is due at the time of service, we do not bill for co-pays.  We will file primary and secondary insurance claims, however, we do not file tertiary claims.  You will need to pay what the secondary insurance says is due and file any tertiary claims for reimbursement directly to you.

 

Acceptable forms of payment are: Cash, Checks, American Express, Discover, MasterCard and Visa.

Cancellation  Policy: A $50.00 fee will be charged when a patient fails to provide us with at least 24 hour notice of cancellation, or is a "No Show" for their appointment.
